Clinical Liaison - Palliative Care Outpatient at Providence TrinityCare Hospice
The Clinical Liaison position is responsible for creating and maintaining positive customer relationships in order to increase referrals for Palliative Care and other services within assigned accounts. This role includes working with discharge planners, facility staff, physicians, and individuals to assess appropriateness for palliative care and hospice services, and facilitate the coordination of services and to ensure there is appropriate documentation for compliance with agency and statutory guidelines to manage the transition of the patient from an acute care facility to the patient's home or next location of residence. Consultative functions include, but are not limited to: providing consultation and informational presentations regarding end of life care and palliative care; sharing expertise through in-services on pain and symptom management options; serving as a clinical resource and educator for physicians, facility staff, patients, families and community on palliative care services.
FTE: 1.0 Days (40 hours a week)
Education sufficient for licensure
Current OT, PT, ST, MSW, LPN, LVN or RN licensure required in the state in which the caregiver resides
Current Driver's license
Current Healthcare Provider CPR certification
Bachelor's Degree: Nursing or other clinical discipline (e.g., OT, Social Work)
3 years Nursing or other clinical discipline (e.g., OT, Social Work)
Liaison, marketing or sales experience
Health care industry experience
Documented prior successful sales account medical management experience
Epic EMR experience
