Registered Nurse Care Manager - Clinical QM $5,000 Sign-On Bonus

Location: Hesperia, CA

Work Schedule: Full Time - 80 Biweekly Hours

Shift: 8-hour, Days

The Care Manager RN - Transition Care position is responsible for providing care transition services for Providence hospitalized patients. The Care Manager works with the Providence physician team and hospital discharge planning staff to ensure patients are successfully discharged from the hospital with no disruption in services. The position encompasses the roles of nursing assessment, liaison abilities, communication, education and team/family support.

Required qualifications:

  • Coursework/Training - Graduate from an accredited school of Nursing.

  • Currently licensed as a RN in the State of California.

Preferred qualifications:

  • Bachelor's Degree in Nursing.

  • 1 year Clinical experience.

  • Experience in providing patient and staff education.

  • Experience in conflict resolution.
