We are looking for a Lead Health Coach with a $1,000 Sign-On Bonus for the SJHH Wellness Center Edwards Life Department at SJHH Wellness Center Edwards Life.

Location: Irvine, CA

Work Schedule: Part Time - 20 Biweekly Hours

Shift: 8-hour, Days

Job Summary:

This position provides health coaching, with an emphasis on nutrition and lifestyle, in the community. The Lead Health Coach functions independently at the wellness center under direction of the medical director in providing group, individual and phone coaching in various settings at the wellness center. The Health Coach will need to work in collaboration and support of onsite clinic providers to develop nutritional meal plans, program and personal nutrition recommendations for clients. In addition, this position is responsible for researching, developing, implementing, tracking and evaluating health improvement programs and outcomes in alignment with St. Joseph Heritage Healthcare's wellness philosophy of care.

Essential Functions:

  • Provide health coaching, including working with employees to establish individualized lifestyle plans, as well as providing motivational support, nutritional guidance and health education to support employees’ efforts to improve their health.

  • Foundational understanding of social cognitive behavioral modification approaches

  • Teach and conduct education sessions in-group and individual settings.

  • Evaluate and assess employee needs and provide appropriate lifestyle planning for patients based on findings.

  • Provide Medical Nutrition Therapy (MNT) and nutritional screenings.

  • Create/author handouts, patient books and other patient education materials to meet the patient needs.

  • Identify and connect employees with programs and resources to support improved health.

  • Track improvements in health behaviors, biometric measurements and health outcomes.

  • Collaborate with wellness team and strategic partners/clients to design, implement and evaluate programs to improve the population health.

  • Provide outcome data to wellness team and partners/clients.

  • Coordinate events to promote services, clinics as well as network providers.


  • Possess knowledge of the principles and practices of dietetic and nutrition.

  • Possess knowledge of the relationship between nutrition, diet and individuals’ physiologic needs.

  • Possess knowledge in planning modified and specialized diets, plans and menus.

  • Ability to estimate food intake requirements to safely achieve individuals’ goals.

  • Ability to read, analyze, and interpret medical records, technical procedures, and related regulations.

  • Ability to document all patient activity and technical information. Ability to effectively present information and respond to questions from physicians, clients, patients, co-workers and outside health care providers.

  • Possess excellence in customer service when dealing with both external and internal persons while on the job.

  • Bilingual English/Vietnamese communication skills preferred.

Minimum Position Requirements:

Education: Bachelor's Degree in Nutrition, Science, Education or related Health field.


  • 1 year working experience as a nutritionist, dietitian or health coach in an outpatient healthcare setting.

  • Experience with providing MNT and individual counseling.

  • Experience with teaching and modeling self-management skills, concepts, resources and tools related to health, lifestyle change and behavior modification to class participants.

  • Experience with working with patients with chronic diseases and other primary prevention programs.

Licenses/Certifications: An active CPR Certificate

Preferred Position Qualifications:

Education: Master's Degree in Nutrition.


  • 2 years working experience as a Nutritionist, Dietitian or Health Coach in an outpatient healthcare setting.

  • AEMR, IDX, Allscripts, PowerPoint and Excel experience.


  • Certified Clinical Nutritionist or Registered Dietitian certified by the Commission on Dietetic Registration as defined by the California Business and professional code and be current with CU/CDR portfolio.

  • Functional Medicine Certified Health Coach.
