Providence Health & Services Clinical Social Worker MSW - Outpatient Palliative Care in Mission Hills, California

Providence is calling a Palliative Care MSW (full time/day shift) to Providence TrinityCare Hospice in Mission Hills, CA

This position is Mon-Fri, no weekends or holidays required, and offers the option to work from home .

We are seeking Palliative Care MSW who will participate in the activities of the interdisciplinary Palliative Care Team. The PC MSW participates in the development and review of appropriate protocols, facilitates formal and informal meetings regarding patient and family communication and decision making processes that support appropriate referral and consistent care, works in collaboration with physicians, hospital, home health and hospice health care teams to provide Palliative Care services in patient, out-patient and in home settings to seriously ill patients and their families. The PC MSW engages in independent decision-making about psychosocial needs, provides psychosocial services to individuals and their families, and provides formal and informal meetings regarding patient and family communication and decision making. The PC MSW obtains a psychosocial history, including chart review and information gathered from the patient and family, and conducts psychosocial palliative care needs assessments. The PC MSW provides direct patient care as well as consultative services to assure the delivery of state-of- the-art palliative care. In addition to these clinical duties, the Palliative Care MSW is responsible for contributing social services work expertise to staff education, data collection, and quality improvement activities as they relate to inpatient, outpatient and home palliative services.

Qualifications:

Required qualifications for this position include:

  • Master's Degree in Social work or equivalent

  • 3 years Inpatient, Outpatient, or home-based Social work

  • Demonstrated evidence of experience in palliative care settings, such as hospice, i.e., ability to:

  • Work independently as well as collaboratively on a small interdisciplinary team

  • Provide and model interdisciplinary palliative assessment and management of physical, psychosocial and spiritual symptoms during aggressive treatment and end of life care

  • Possesses a working knowledge of comprehensive palliative care

  • Knowledge of MSW scope of practice

  • Knowledge of stages of dying/death and age-appropriate outcomes and interventions

  • Knowledge of basic pain and symptom assessment

  • Knowledge of infection Control principles and practice

  • Understands role as a clinical leader

  • Functions well with an interdisciplinary team

  • Time management skills.

  • Critical thinking and prioritization skills

  • Communication and interpersonal skills

  • Knowledge of referral networks

  • Documentation skills

  • Knowledge of quality improvement processes

  • Knowledge of cultural differences and culturally appropriate outcomes and interventions

  • Teaching skills and knowledge of learning theory and learning styles

  • Knowledge of patient education principles

  • Customer service knowledge and skills

  • Knowledge of confidentiality policy

Preferred qualifications for this position include:

  • LCSW or equivalent

  • Bilingual in Spanish/English a plus

  • Flexibility in assignments
