Providence is calling a Discharge Planner (Per Diem/Day Shift) to Providence Facey Medical Foundation in Mission Hills, CA.

We are seeking a Discharge Planner who will collaboratively work with RN/LVN case manager, to manage and coordinate all non-clinical duties related to care of members who have complex medical problems requiring multidisciplinary intervention on weekdays, weekends and holidays. The purpose is to coordinate, facilitate, and follow a patient's use of an array of health and social services by utilizing community based services to prevent over-utilization and re-admissions by meeting the member's needs during and after hospitalization.

In this position you will:

  • Deliver upon the service expectations of both our patients and fellow staff members by listening to their needs; engaging in positive interactions; and following through on promises made in a thoughtful, efficient, timely and courteous manner so that their total outcome is better than expected

  • Respect the dignity, confidentiality and privacy of patients

  • Work in a safe manner, adhering to general safety precautions and standards

  • Report any unsafe conditions to their supervisor and/or the safety hotline

  • Be helpful to patients/visitors who need assistance in finding their way around the facility

  • Adhere to established policies contained in the Human Resources and internal departmental manuals

  • Complaint are referred to the UM Discharge Planner Supervisor if the problem cannot be immediately solved

  • Physicians and fellow staff members are treated courteously

  • Respond to requests from other departments in a timely, positive and friendly manner

  • ActIvely promote communication between all team members, family, member, provider and all parties involved

  • Obtain daily hospital census information and coordinate with Physicians and UM Case Manager

  • Enter referrals thru MSO UM Referral System as needed

  • Determine appropriate providers for DME, infusion, home health, hospice, skilled nursing, ambulatory, consultants with hospital, and ambulances, etc.

  • Fax orders and phone intake to all providers/agencies

  • Research all needed information in MSO UM Referral System, EPIC and various EHR’s of hospital/medical groups

  • Make appointments: follow up with PCP and/or specialist and radiology procedures

  • Schedule PCP appointments in EPIC

  • Verification of insurances (HMO, Full Risk patients, Medicare, PPO, etc.)

  • Check benefits for all required referrals (SNF, HH, HOSPICE, DME)

  • Arrange outpatient services: DME, HH, SNF, therapy, PCP follow up, specialist, education, and care management

  • Make post discharge telephonic follow up call within 1 week of discharge and document in EPIC

  • Report any issue to UM Discharge Planner Supervisor

  • May assist case manager gathering all relevant data and obtains information by interviewing member/family

  • Assist case manager to work with the member/family to develop a discharge plan which enhances member outcome and improve utilization of services across the continuum, The planning process includes member/family as the primary decision maker and goal setter

  • Discharge patients from MSO UM Referral System with appropriate diagnosis and level of care

  • Act as liaison between UM staff and hospital case managers

  • Attend monthly UM Staff meetings

  • Report promptly any suspected or potential violations to laws, regulations, procedures, policies and practices, and cooperates in investigations

  • Conduct all transactions in compliance with all company policies, procedures, standards and practices

  • Demonstrate knowledge of all applicable compliance and legal requirements of the job based on the scope of practice of the position

  • Ensure that appearance and personal conduct are professional at all times

  • Excellent attendance record

  • Wear appropriate clothing for job functions

  • Work at maintaining a good rapport and a cooperative working relationship with staff and physician offices

  • Represent the organization in a positive and professional manner in the community

  • Maintain organizational, employee and patient confidentiality at all times

Qualifications:

Required qualifications for this position includes:

  • One (1) year current medical office or utilization management experience

  • Basic knowledge of disease processes

  • Able to recognize problems and to use effective judgment in problem solving

  • Demonstrate proper phone etiquette, strong computer skills including knowledge of EPIC, MSO UM Referral System, MS Outlook, Word and Excel

Preferred qualifications for this position include:

  • Verified skill sets defined by department usage
