Providence RN Navigator Oncology in Mission Hills, California


Providence is calling an Oncology Nurse Navigator (RN) (Full time, Night Shift) to Providence Holy Cross Medical Center in Mission Hills, CA.

We are seeking an Oncology Nurse Navigator position that will be responsible for facilitating all activities and services across the continuum of care for oncology patients. Responsible for coordinating and liaising to design and align resources to support program initiatives, program development, development and maintenance of relationships with internal and external referring physicians, and community resources. Performs all duties in a manner which promotes team concept and reflects the mission and values of Providence Health & Services

In this position you will have the following responsibilities:

  • Serve as an advocate to patient/caregiver.

  • In collaboration with the patient/care giver, plan and coordinate all aspects of care for the patient throughout the continuum of services – from initial referral to follow-up care.

  • Empower patient/care giver to actively participate in the development of the plan of care.

  • Assist patient with issues related to treatment goals, advance directives, palliative care and end of life concerns using an

  • ethical framework that is non-judgmental and is non-discriminatory.

  • Facilitates individualized care within the context of functional status, cultural consideration, health literacy, psychosocial, reproductive/fertility and spiritual needs for patients, families and care givers.

  • Identifies potential barriers to care and implement actions to mitigate by collaboration with the interdisciplinary team.

  • Facilitates care conferences as indicated by complexity of patient’s clinical condition/medical management.

  • Evaluate patient’s ability to follow directions/instructions and give consent.

  • Provide necessary education and information on available options, the informed decision making process, and realistic goal setting.

  • Provide and reinforce education about diagnosis, treatment options, side effect management and post treatment care and survivorship.

  • Coordinate, implement and evaluate educational programs/projects for patients and the community at large.

  • Assist in the identification of candidates for molecular testing and/or genetic testing and counseling and facilitates appropriate referrals and provide education.

  • Referral of potential research patients to clinical trial navigator.

  • Maintain competency skills including ability to provide care to special patient populations and patients with diverse cultural backgrounds.

  • Participate in development of guidelines, protocols, pathways and other performance improvement activities.

  • Participate in the tracking and monitoring of metrics and outcomes of the navigation program and report findings to the cancer committee.


Required qualifications for this position include:

  • RN – current California Registered Nurse License.

  • BLS for Healthcare Provider sponsored by the American Heart Association.

  • Bachelor’s degree in nursing

  • 5 years Nursing experience, including 3 years in Oncology

  • Possess strong interpersonal skills

  • Ability to effectively communicate with patients, customers, co-worker or leaders via written and/or verbal

  • Working knowledge of national, regional and community resources specific to oncology patients and their needs.

  • Ability to remain flexible in response to change in work demands, varying projects and time lines.

  • Ability to work independently and seek consultation when appropriate.

  • Ability to work with multiple departments, hospitals, and their leaders.

  • Ability to work under pressure in a fast paced sometimes stressful work environment.

  • Solid computer skills (proficiency in Microsoft Outlook, Word, Excel, and PowerPoint).

Preferred qualifications for this position include:

  • Master’s Degree in Nursing

  • Oncology certification upon hire preferred, required within 1 year of hire (Oncology Nurse Navigator, OCN, AOCN, AOCNP, CBCN).
