This position will be traveling to see patients in the South Bay and/or Long Beach areas.

RN - Home Health

The professional nurse applies the nursing process to develop and execute an age-appropriate plan of care for assigned home care/hospice patients by coordinating the interdisciplinary team to implement and/or update the patient plan of care according to assessment findings.

Required qualifications:

  • MOAB I within 30 to 150 days of hire

  • Current California Licensed Registered Nurse

  • Current BLS HCP Basic Life Support for Healthcare Providers American Heart Association (AHA) or American Red Cross (ARC) - CPR/AED for Professional Rescuer accepted on hire until card expiration date

  • Current California Driver License

  • 1 year professional nursing experience

  • Experience in cardiac care (for Staff on the Cardiac team

  • 1 year experience in Maternal and Child Nursing (for Staff on the Maternal and Child team)

About the home health ministry you will serve:

As an integral part of the respected Providence Health & Services for over 25 years, our highly skilled healthcare professionals provide a full range of home health and clinical specialty programs to patients experiencing an illness, hospitalization or surgery or, for those receiving treatment for a chronic or acute illness. Providence Home Health embraces everyone from new mothers and their babies to the elderly with chronic health problems.

Our team of health care professionals starts with the patient and his or her physician and may include: registered nurses; certified home health aides; social workers; physical, occupational and speech therapists. Our specialty care includes patient assessment and monitoring; beside protime monitoring; IV therapy; total parenteral nutrition; gastric feeding tube (G-Tube) for long-term enteral nutrition; V.A.C. therapy used to promote healing in acute or chronic wounds, fight infection and enhance healing of burns; pain management; injections; and ostomy management. From same-day referrals and a smooth transition from hospital to home, to attentive follow-up and communication with your own doctor, we provide care and support for you every step of the way to help restore your health to its fullest potential. About the Team

Providence Home and Community Care (HCC) is a service line within Providence with over 8,000 caregivers and 2,000 volunteers serving nearly 200,000 people a year, in their homes, in clinics and in a variety of congregate living ministries across our seven-state footprint.

HCC provides a full range of services and support for people of all ages, especially those facing chronic or life-limiting illnesses. Our core competencies include restorative care, longitudinal care and care through the end of life. Our service lines include assisted living/skilled nursing/rehabilitation, home health, home infusion/pharmacy services, home medical equipment, hospice and palliative care, Program of All-Inclusive Care for the Elderly (PACE), personal home services/private duty care and supportive housing.
