We are seeking a Community Health Worker Academy Intern for an approximate 6 month internship who will be responsible for helping community residents and Medical Center patients and their families to navigate and access community services, other resources, and adopt healthy behaviors. The CHW Academy Intern provides primary job duties within at least one of the following tracks:

a) Enrollment: CHW Academy Interns in this track will provide initial screening for eligibility of Covered CA, Medi-Cal, and CalFresh (Food Stamps). In addition they will provide one-on-one health insurance and/or CalFresh enrollment application assistance.

b) Navigation/Linkage to Resources: CHW Academy Interns in this track will provide individualized assessments in clinical settings such as emergency departments, primary care clinics, or in a community-based health center setting, then referring them to the appropriate programs and resources, and providing follow-up calls to make sure the needs are met. This includes placing follow up calls to patients tested positive for COVID-19 to ensure they contact their primary care provider for further recommendations concerning treatment and level of care.

In this position you will:

  • Navigation/Linkage to Resources Duties (for CHW Academy Interns in the navigation/linkage track):

  • Provide on-site screenings of risk-factors that impact an individual’s well-being in community and Medical Center settings. For example, screening for access to health care, food, housing, etc.

  • Refer clients/patients to the appropriate programs in the community that meet the needs identified in the screenings. For example, community health programs for diabetics or other chronic conditions, housing programs, Covid-19 community resources, etc.

  • Follow up with patients by telephone after Covid-19 testing to connect him/her for their medical appointments and/or next steps.

  • Build relationships with clients/patients by asking appropriate questions in a compassionate manner and uses good listening skills over the phone or in person.

  • In Medical Center settings enroll patients into Hospital Presumptive Eligibility (HPE) and provide enrollment assistance or refer patients to Children’s Health Insurance Program (CHIP) for patients that need to be enrolled in Covered CA, Medi-Cal, and/or CalFresh.

  • Follow up and assist families who are having difficulty accessing resources to which they were referred. In addition, assist by modeling how to navigate the systems in order to learn independently for patients, families, and their community.

  • Administer client satisfaction survey to clients assisted.

  • Fully understand and is able to explain the issues for which they are screening and the resources available to meet those needs.

  • Enrollment Duties (for CHW Academy Interns in the enrollment track):

  • Provide outreach to increase awareness of Covered CA, Medi-Cal, and CalFresh.

  • Administer screening tool to identity and refer community members/patients who are eligible for Covered CA, Medi-Cal and/or CalFresh.

  • Provide on-site and/or community-based assistance of enrollment and renewal applications to families.

  • Build relationships with patients/clients by asking appropriate questions in a compassionate manner and uses good listening skills.

  • Follow up and assists families who are having difficulty with enrollment process.

  • Conduct proactive community-based follow up with families to make sure they stay enrolled and renew their application on time.

  • Administer client satisfaction survey to clients assisted.

  • Fully understand and can explain insurance and CalFresh programs (eligibility, benefits, application process, requirements, premiums, co-pays) and other community health services.

  • Maintain professional and technical knowledge by attending educational workshops or reviewing professional publications to stay informed of eligibility and benefit changes to insurance programs. For example, immigration regulations such as public charge expansion.

  • Enrollment Duties (for CHWs in outreach and enrollment track):

  • Provide outreach in the community to increase awareness of Covered CA, Medi-Cal, and CalFresh.

  • Administer screening tool to identity and refer community members/patients who are eligible for Covered CA, Medi-Cal and/or CalFresh.

  • Provide on-site and home-based assistance of enrollment and renewal applications to families from partner organizations (clinics, schools, churches, businesses, etc.).

  • Build relationships with patients/clients by asking appropriate questions in a compassionate manner and uses good listening skills.

  • Follow up and assist families who are having difficulty with enrollment process.

  • Conduct proactive community based follow up with families to make sure they stay enrolled and renew their application on time.

  • Administer client satisfaction survey to clients assisted.

  • Fully understand and is able to explain insurance and CalFresh programs (eligibility, benefits, application process, requirements, premiums, co-pays) and other community health services.

  • Maintain professional and technical knowledge by attending educational workshops or reviewing professional publications to stay informed of eligibility and benefit changes to insurance programs.

  • Work closely with Community Health enrollment and emergency room caregivers to assure a seamless flow for qualified uninsured adults to gain access to Medi Cal or Covered CA.

  • Troubleshoot complex cases independently.

  • Provide small group presentations on how to use insurance and basic health care rights.

  • Navigation/Linkage to Resources Duties (for CHWs in the navigation/linkage track):

  • Manage complex patient subpopulations, including but not limited to people with co-morbidities, recently discharged patients from the Medical Centers or community residents struggling with substance abuse.

  • Work collaboratively and professionally with a Social Worker when necessary to manage complex patient subpopulations.

  • Provide on-site screenings of risk-factors that impact an individual’s well-being in community and Medical Center settings.

  • Refer clients/patients to the appropriate programs in the community that meet the needs identified in the screenings.

  • Build relationships with clients/patients by asking appropriate questions in a compassionate manner and uses good listening skills.

  • In Medical Center settings enroll patients into Hospital Presumptive Eligibility (HPE) and provide enrollment assistance or refer patients to CHIP for patients that need to be enrolled in Covered CA or CalFresh.

  • Work closely with Community Health and Medical Center caregivers to ensure a seamless flow for the patients.

  • Documentation/Information Management Duties:

  • Document all direct services and referrals including whether a client successfully accessed a referred service.

  • Implement and monitor collection of program’s evaluation data (e.g. surveys).

  • Ensure that program statistics are entered into the appropriate database per department deadlines.

  • Submit monthly reports to Supervisor.

Qualifications:

Required qualifications for this position include:

  • An interest in the health care field and becoming a Community Health Worker or equivalent educ/experience

  • CA driver’s license

  • 1 year experience working with the public or with community groups in performing duties such as interviewing clients or patients concerning health or social service matters, answering questions, and giving information about health, mental health, and/or social services to clients or patients

  • Experience working in a health care setting such as a clinic, health center, hospital, etc.

  • Ability to work collaboratively with others and to participate fully in a team process.

  • Strong presentation skills

  • Excellent interpersonal skills

  • Ability to complete necessary tasks and meet deadlines

  • Ability to follow directions, be a self-starter and collaborate with coworkers

  • Willingness to learn computer systems and programs

  • Efficient word processing/typing skills

  • Proficient technical skills in using Microsoft Office Suite software

Preferred qualifications for this position include:

  • Bi-lingual English /Spanish
