Providence is calling a Full Time, Variable shift Community Health Worker II to Providence Little Company of Mary Medical Center in San Pedro, CA.
In this position you will:
Beresponsibleforhelping community residents and Medical Centerpatientsandtheirfamiliesto navigateandaccess community services,other resources,andadopt healthybehaviors.
The CHW-II l provides primary job duties within at least one of thefollowing tracks:
Health Education: CHWs in this track will assist withrecruitment and administrative tasks for health education courses, such as butnot limited to Mental Health First Aid, Creating Healthier Attitudes Today,physical activity, or Groceryships. In addition they will provide modules thesehealth education topics with oversight from a Health Educator or PhysicalEducation Specialist.
Outreach and Enrollment: CHWs in this track will provideoutreach in community settings and provide initial screening for eligibility ofCovered CA, Medi-Cal, and CalFresh (Food Stamps). In addition they will provideone-on-one health insurance and/or CalFresh enrollment application assistance.
Navigation/Linkage to Resources: CHWs in this trackwill provide individualized assessments of risk factors that impact anindividual’s well-being. They will beresponsible for doing the assessments in community and Medical Center EmergencyDepartment settings and then referring them to the appropriate programs andresources in the community.
Required qualifications for this position include:
Minimum six monthsexperience in a Community Health Worker I position or one year related communityoutreach, enrollment, education or other community health administrativeexperience required.
ForCHWs providing outreach and enrollment, become a Certified Enrollment Counselor (CEC) and be certified to enroll into Cal Fresh within 3 months of hire.
Willingnessto learn computer systems and programs
Accurate word processing/typing skills.
Preferred qualifications for this position include:
Minimum high school diploma highly preferred
BilingualEnglish /Spanish, highly preferred
