Providence Community Health Worker II in San Pedro, California
Providence St. Joseph Health is calling Temporary Community Health Worker II (On Call, Variables) to our location in San Pedro, CA.
We are seeking a Community Health Worker to be responsible for helping community residents and Medical Center patients and their families to navigate and access community services, other resources, and adopt healthy behaviors. The CHW-II will provide job duties within at least one of the following tracks:
In this position you will have the following responsibilities:
IHealth Education: CHWs in this track will assist with recruitment and administrative tasks for health education courses, such as but not limited to Mental Health First Aid, Creating Healthier Attitudes Today, physical activity, or FEAST. In addition they will provide modules these health education topics with oversight from a Health Educator or Physical Education Specialist.
Outreach and Enrollment: CHWs in this track will provide outreach in community settings and provide initial screening for eligibility of Covered CA, Medi-Cal, and CalFresh (Food Stamps). In addition they will provide one-on-one health insurance and/or CalFresh enrollment application assistance.
Navigation/Linkage to Resources: CHWs in this track will provide individualized assessments of risk factors that impact an individual's well-being. They will be responsible for doing the assessments in community and Medical Center Emergency Department settings and then referring them to the appropriate programs and resources in the community.
Required qualifications for this position include:
2 years experience working and/or volunteering in the community highly
Valid driver's license
For CHWs providing outreach and enrollment, become a Certified Enrollment Counselor (CEC) and be certified to enroll into Cal Fresh within 3 months of hire
Preferred qualification for this position include:
- H.S. Diploma or GED
