We are seeking a LVN- Sub Acute who will implement delegated patient care functions under the direction of the Professional Registered Nurse. This individual will perform these functions according to established hospital standards of nursing care, standards of nursing practice and within the Vocational Nurse Practice Act. Respond appropriately to patients and families with knowledge of age-specific needs based on scope of service of unit/department where assigned.
In this position you will:
Administer medications including controlled substances within scope of license
Start and add IV fluids in accordance with hospital policies and procedures and Vocational Nurse Practice Act
Discontinue peripheral IV’s and PRN adapters
Assist with patient admission/transfer, to include room set-up, orientation of patient to environment, personal effects labeling and storing
Provide patient/family education related to self-care and supporting the Plan of Care
Assist with patient discharge related to self-care and supporting the Plan of Care
Provide direct patient care in a safe and caring manner, as delegated by the Patient Care Leader (RN):
Perform data collection through physical observation to include auscultation, palpation and percussion and basic mental status assessment to include observation of general behavior, appearance and attitude, collection of data relative to sensorium or orientation to time, person and place, and assessment of recall ability
Place, change and remove complex and simple dressings
Insert/remove foley catheters. Observe and supervise patients with incentive spirometry
Apply low flow O2 therapy and simple mask
Insert nasogastric tube assessing for correct placement
Perform tracheal suctioning and routine tracheostomy care
Operate/administer meter dose inhalers
Initiate tube feedings as directed by the R.N.
Take and record vital signs, weight, etc., as assigned
Perform/record point of care tests
Collect, accurately label, record phlebotomy specimens according to established protocols
Apply, change, remove external catheters
Perform pre-surgical preps as needed (i.e. shave)
Ambulate, re-positions patient and performs ROM
Apply changes and remove patient restraints according to established policy
Apply K-pads, cold packs and hyper/hypothermic blankets
Empty and accurately records fluid collection devices
Perform EKG’s
Perform tray/supply set-up for a bedside procedure
Perform monitoring activities, reports, documents (i.e. dynamap, pulse ox, fetal and telemetry)
Provide post-mortem care
Assist in complex feeds utilizing approved practices
Assist in IV set-up (gathering supplies, pump, etc.)
Assist with orthopedic equipment/applications as directed
Provide personal hygiene care
Perform transportation of patients, lifting support as assigned
Manage CPM device
Check patient’s ID band every shift
Leadership and Supervision:
Act as a team problem solver by being an active member of unit/staff meetings and other committees
Assist with the orientation and on-the-job training of new hires
Perform within own legal scope of practice
Documentation/Information Management:
Maintain confidentiality of all patient and hospital related information
Demonstrate competency/working knowledge of Hospital Information System
Perform documentation on unit records and in patient’s record, which is timely, accurate, and concise
Utilize and document on patient data base and other computer based communication and patient monitoring systems (e.g. EKG, Pulse Oximeter, hemodynamic monitoring)
Communicate patient information/changes to RN and to physician when appropriate
Take verbal/telephone orders from physician according to hospital policy and procedure
Enter and verify MD orders accurately
Obtain results on diagnostic data in a timely manner and communicate and files appropriately
Enter requisitions in a timely manner
Performance Improvement/Quality and Safety:
Actively participate in performance improvement activities and works to correct deficiencies identified
Actively support the facility with regards to continuous compliance with all regulatory and performance improvement standards including, but not limited to JCAHO, Department of Health Services, CMS Core Measures, IHI initiatives, etc
Maintain current knowledge regarding JCAHO standards for this position
Commit to and actively participates in ongoing compliance
Have knowledge of Tracer methodology
Provide the best care and service to every person, every time (PHS Quality Vision)
Provide care that is safe, timely, effective, efficient, equitable and patient/family centered (PHS Quality definition)
Qualifications:
Required qualifications for this position includes:
Completion of program for Licensed Vocational Nurse
Current Licensed Vocational Nurse licensure in good standing in California
Satisfactory completion of BLS
Able to perform all physical requirements pertaining to delegated tasks
Able to accurately and efficiently complete assigned tasks
Able to organize and prioritize work to complete assignments in a timely manner
Able to establish and maintain a courteous and cooperative (service-oriented) working relationship with patients, families and co-workers
Demonstrate competency in medication administration
Preferred qualifications for this position include:
Successful completion of course in Intravenous Therapy approved by the LVN Board
Hospital direct patient care experience on an acute, sub-acute unit or skilled nursing facility
One (1) plus years of acute care institutional experience
