We are seeking a Manager Welcome Baby Program who will have overall responsibility for the implementation of the Welcome Baby program. The Program Manager must work effectively as part of a multidisciplinary team that includes the hospital, other Welcome Baby sites in the community, community stakeholders, Best Start Partnership, Oversight Entity, and First 5 LA. This position has a direct reporting function to the Nurse Manager, Maternal Child Health. This position supervises the Clinical Project Manager, the Parent Coach Supervisor and Parent Coaches, the Welcome Baby Nurses, the Hospital Liaisons, and Outreach Specialist.

In this position you will:

  • Operations: Responsible for facilitating services provided by an interdisciplinary team and managing Project activities within the parameters of an approved budget, including:

  • Assist in maintaining and improving the quality of the program by ensuring that the following is conducted by the management team:

  • Group reflective supervision

  • Individual reflective supervision of home visitation staff

  • Periodic shadowing of staff on home visits

  • Regular chart reviews to ensure the accurate collection of data and completion of all client documentation in a timely and organized manner

  • Work with staff individually, and in a group setting, to plan, implement, evaluate and improve the delivery of Project services

  • Establish goals and objectives for each employee and give ongoing feedback to employees on their performance

  • Work closely with Welcome Baby RN

  • Assess strengths and areas of improvement for each employee and communicate observations

  • Is responsible for ensuring Welcome Baby deliverables are met as defined in the Scope of Work

  • Oversee and monitor program to ensure standardized implementation, including adherence to protocols, policies, procedures and documentation

  • Coordinate with Grant Accountant to maintain accurate files for expenditures including copies of receipts, invoices, documents and products

  • Assist with fiscal monitoring of program and help formulate annual budget and needed budget modifications throughout the year

  • New Project Implementation/Planning/Outreach:

  • Monitor program implementation for ongoing problems and barriers and identifies possible solutions

  • Proactive in reaching out and working closely with local medical/clinic providers, churches, and formal/informal social networks to develop a comprehensive referral network

  • Monitor and track any gaps or barriers to clients being able to access needed services identified by post partum assessment or at home visit

  • Participate in local community meetings, and develop relationships with providers and community organizations to enhance partnerships and collaboration as appropriate

  • Understand the needs of the community and collaborates with local agencies to provide needed services. Identifies gap in resources and attempts to find solutions through advocacy and collaboration

  • Work closely with the Outreach Specialist and Parent Coaches conducting outreach to monitor and improve prenatal outreach, acceptance and enrollment

  • Attend all Welcome Baby/ First 5 required trainings

  • Ensure that all trainings are completed as required by First 5

  • Collaborate with First 5 LA and Oversight Entity in marketing efforts

  • Remain compliant with First 5 Compliance Guidelines and Contract

  • Evaluation:

  • Monitor progress in achieving programmatic goals and objectives within the Scope of Work

  • Submit progress reports and invoices within time frames set up by funding agency

  • Share key program data with staff, partners and other stakeholders on a quarterly basis, including outcomes, demographics, and enrollment information

  • In the event of a site visit by First 5 LA, provides Program or Contract Compliance Officer with requested documents, activities and arranges for staff interviews

  • Evaluate program effectiveness and provide documentation regarding progress by maintaining accurate, complete and correct records

  • Collaborate and cooperates with the Oversight Entity and First 5 LA to assist with data and evaluation needs

  • Communication:

  • Establish a communication plan to community that encompasses key elements of Welcome Baby Project and PHCMC’s role in the project

  • Establish a relationship and visibility for the program with key personnel at the hospital, including Women’s Pavilion and Social Work staff

  • Work collaboratively with Maternal Child Health management team and staff to ensure Welcome Baby goals and data are communicated regularly, barriers are addressed and that there is two-way communication

  • Attend and participates in funding agencies' meetings, webinars and conference calls

  • Annually identifies, with supervisor input, opportunities for program development and develops goals and objectives which expand the scope and/or quality of program services

  • Establish and maintain positive communication with supervisor

  • Collaborate with PHCMC administrative Departments including Marketing and Foundation to design, develop story line and approve stories/information to be disseminated to the public and for presentation to community coalitions

  • Development:

  • Annually identifies, with supervisor input, opportunities for further professional development related to models and styles of supervision as well as latest trends in field of expertise


Required qualifications for this position includes:

  • Bachelor's Degree

  • Valid California driver’s license

  • Specific experience in the ability to analyze information and develop strategies that are designed to accomplish defined goals

  • Three (3) years of Program administration and supervisory experience in a healthcare or community based organization

  • Three (3) years of Program experience in a healthcare or community based organization which serves the needs of under served children and/or adults

  • Demonstrated commitment to the development of collaborative services and improved access to health care for vulnerable populations

  • Proven program development and planning skills

  • Able to give constructive feedback to staff, consultants, and community stakeholders and facilitate an efficient, team oriented work environment

  • Be knowledgeable regarding contract and compliance guidelines

  • Bilingual skill in Spanish is a plus

  • Strong public speaking and presentation skills

  • Strong organization and time management skills

  • Willingness to travel frequently in local area and to conferences sponsored by funding agency

Preferred qualifications for this position includes:

  • Master's Degree in Public Health, Social Work, Healthcare Administration, Nursing, or related field

  • Certificated Lactation Educator training completion within six (6) months of hire

  • 5 years experience in Maternal Child Health

  • Case management and familiarity with home visit and/or pregnancy case management program experience

  • Proficiency with word processing software required, familiarity with spreadsheet software

  • Bilingual in English/Spanish
