Behavioral Health Care Manager Collaborative Care Model (LCSW)
The behavioral health care manager is a core member of the collaborative care team, including the patient's medical provider and psychiatric consultant, as well as the larger primary care team or medical team. The behavioral health care manager is responsible for supporting and coordinating the mental and physical health care of patients on an assigned patient caseload with the patient's medical provider and, when appropriate, other mental health providers.
Participates in the development and review of appropriate protocols, facilitates formal and informal meetings regarding patient communication and decision making processes that support appropriate behavioral health care. Works in collaboration with physicians and the interdisciplinary care teams to provide services to patients with behavioral health needs.
Age(s) of Patients Served: 12 years and above.
Qualifications:
Required qualifications for this position includes:
Bachelor's Degree
Master's Degree in Social Work
Licensed Clinical Social Worker in the State of California
Complete all initial and annual required learning relevant to licensure and this role
2 years of Inpatient, outpatient, or home-based social work
Demonstrated experience in counseling and group work
Experience with screening for common mental health and/or substance abuse disorders
Experience with assessment and treatment planning for common mental health and/or substance use disorders
Experience with evidence-based counseling techniques
Preferred qualifications for this position includes:
Spanish speaking
Knowledge of the legal issues arising from illness and patient care issues
Excellent verbal and written communication skills, Microsoft suite, e.g. Excel, Word, etc
Working knowledge of computer applications, i.e. Microsoft Office
Complies with all relevant laws, regulations, and policies
Demonstrated ability to collaborate and communicate effectively in a team setting
Ability to maintain effective and professional relationships with patient and other members of the care team
Working knowledge of differential diagnosis of common mental health and/or substance abuse disorders, when appropriate
Ability to effectively engage patients in a therapeutic relationship, when appropriate
Ability to work with patients by telephone as well as in person
Working knowledge of evidence-based psychosocial treatments and brief behavioral interventions for common mental health disorders, when appropriate (e.g., motivational interviewing, problem-solving treatment, behavioral activation)
Basic knowledge of psychopharmacology for common mental health disorders that is within appropriate scope of practice for type of provider filling role
Ability to interpret verbal and non-verbal communication
Knowledge of MSW scope of practice
Ability to work effectively with diverse populations
Understands role as a clinical leader
Functions well with an interdisciplinary team
Time management skills
Critical thinking and prioritization skills
Communication and interpersonal skills
Documentation skills in EHR and Microsoft suite, e.g. Excel, Word, PPT etc
Knowledge of quality improvement processes
Knowledge of cultural differences and culturally appropriate outcomes and interventions
Teaching skills and knowledge of learning theory and learning styles
Knowledge of patient education principles
Customer service knowledge and skills
Knowledge of confidentiality policy
