Providence is calling a Licensed Clinical Social Worker for Full-Time Day Shift at Providence Saint John's Health Center, in Santa Monica, CA
We are seeking a Clinical Social Worker who will be under the general supervision of the Director, Case Management, the Licensed Clinical Social Worker provides assessment, counseling and information and referral to patients, outpatients and their families. The Clinical Social Worker works cooperatively with medical and paramedical staff.
In coordination with the Director of the John Wayne Cancer Center Clinics, the LCSW is available to the Radiation Oncology Department, Margie Person Breast Center Clinic and John Wayne Cancer Center Clinics to provide psychosocial services to cancer patients and their families.
The social worker may follow patients to the in-patient unit as determined appropriate clinically and in close coordination with the in-patient medical social work team. The LCSW leads the out-patient team's development in the management plan for cancer patients who live at a distance, have literacy barriers and limited resources.
In this position you will:
Perform assessments, including psychosocial, and data collection
Formulate goal directed plan of care based on patient outcomes
Collaborate with other services
Identify gaps in patient’s care plan which may include transportation, lodging, finances, and ongoing mental health support
Advocate for out-patient services as desired by patient and in accordance with medical team recommendations
Implement care based on department and hospital standar
Participate in education opportunities to further knowledge base
Document patient care records as appropriate
Identify patient and family learning needs and demonstrates measures to meet these needs
Apply skills and demonstrates competency in selected skills including, but not limited to:
Perform crisis intervention
Counsel individual patients and their families
Provide patient and family education
Consult with medical, nursing and ancillary staff regarding psychotherapeutic issues of patients
Discuss end-of-life planning, educates on advance directives and palliative care as an additional support.
Facilitate appropriate linkages between patient needs and community resources, including issues related to transportation for treatments
Comply with hospital, department and unit standards including, but not limited to, safety, CPR, infection control, quality management, environmental, confidentiality, and patient care
Conduct groups as developed in coordination with Lead Clinical Social Worker and Director of Care Coordina
Demonstrate responsibility and accountability for own professional practice in regard to:
Attendance, punctuality and patient confidentiality
Positive public relation image and working relationships
Obtain necessary knowledge and skill needed to maintain competence and certifications
Complete all mandatory requirements, including annual safety programs, CPR, infection control and annual physical
Participate in the development and attainment of team, department and hospital goals including:
Attend and participate in division meetings
Volunteer or accept assignments for projects
Mission, vision and team ground rules
Maintain Standard 3.2 Psychosocial Services and Distress Screening protocols
Contribute to Navigation and Survivorship Program
Report to Cancer Committee bi-monthly
Qualifications:
Required qualifications for this position include:
Master's Degree in Social Work (MSW) from an accredited school
Licensed Clinical Social Work (LCSW) by California Board of Behavioral Science
Ability to perform rapid, accurate psychosocial assessments
Knowledge of mandatory reporting requirements
Knowledge of crisis intervention techniques
Knowledge of issues common to oncology patients and their families
Ability to deal effectively with many people across a broad range of cultural backgrounds and ages
Ability to work independently with little or no supervision
Proficient in P.C. software applications including, Microsoft Word, Excel, PowerPoint, Access, Outlook, etc
Ability to prioritize, organize and coordinate work.
Ability to work effectively with many people at various levels of the organization
Excellent oral and written communication skills
Ability to handle multiple tasks and appropriately prioritize
Ability to clearly communicate with internal and external customers, including, but not limited to, patients, co-workers, management, physicians, and visitor
Ability to maintain patient confidentiality and privacy in accordance to hospital policy
Willingness to obtain certifications and provide supervision for MSW staff and MSW students
Ability to use specialized interventions and treatment modalities including group and individual supportive counseling
Knowledge in developing and implementing methods for measuring effectiveness of social work practice and in using outcome evaluations to improve treatment services
Ability to design system changes based upon empirical findings
Preferred qualifications for this position includes:
3 years Post M.S.W. work
Medical clinics and acute medical setting (including field work pre-M.S.W.)
Topics and treatments related to oncology
Certification as Oncology social worker (LCOSW)
If LCOSW is not complete, the expectation in certification within the first year of employment
Approved Social Worker (ASW)
