We are seeking a Physical Therapist ( PT ) in Dept who will support the mission, vision, values and objective of Saint John's Health Center. Responsibilities include but are not limited to discharge planning, performing patient evaluations, establishing and implementing a plan of care, monitoring patient's progress. Demonstrate professional communication and interpersonal skills including direct, honest and compassionate behavior.
In this position you will:
Facilitate teamwork and effective flow of ideas by participating in an environment of trust characterized by openness, honesty, and fairness
Facilitate good working relationships between physicians, staff, and administration. Maintains a high level of professionalism when working with others
Participate in the provision of care by:
Perform patient assessments/data collection
Formulate goal directed plans of care based on patient outcomes
Initiate, individualize, and modify plan of care
Collaborate with other services
Implement care based on department and hospital standards
Follow established protocols and procedures
Evaluate effectiveness of care
Identify patient/significant other learning needs and demonstrating measures to meet these.
Delegate to appropriate patient care team members
Demonstrate applied knowledge when caring for selected patient populations including care appropriate to the age of patients served
Demonstrate competency in select skills including, but not limited to:
Perform evaluations such as wounds, strength, sensation, gait, balance and function
Perform techniques such as gait training, transfer training, therapeutic exercise, balance and neuromuscular re-education
Lymphedema management and rehabilitation of breast cancer patients
Utilize equipment such as ultrasound, electrical stimulation devices, TENS and whirlpools
Practice in a professional, collaborative, and proactive manner in all interactions with patients, visitors, physicians and staff
Demonstrate flexibility adjusting to changes in patient acuity and workload
Participate in committees and special projects
Identify risk management issues, taking appropriate actions, and communicating unresolved issues to the appropriate personnel
Practice cost effective methods in the delivery of care:
Use available resources efficiently
Demonstrate understanding of overall unit needs and using downtime to assist others
Demonstrate responsibility and accountability for own performance and departments performance in regards to:
Attendance and punctuality
Positive public relations image and working relationships
Obtain necessary knowledge and skill needed to maintain competence
Complete all mandatory requirements including current licensure and BLS
Meet regulatory compliance standards (e.g. JCAHO, Title 22, and HIPPA.)
Qualifications:
Required qualifications for this position include:
Bachelor's Degree with certificate from an accredited program in Physical Therapy
Certificate from an accredited program in Physical Therapy
Current Basic Life Support certification (BLS) through the American Heart Association or American Red Cross
Valid PT license
Computer literate (Word)
Physical therapy modalities such as electrical stimulation, TENS, ultrasound etc
Preferred qualifications for this position include:
Experience in Acute hospital setting
Experience in MediTech
