Providence RN Navigator Breast Center in Santa Monica, California

Description:

Providence is calling a RN Navigator for Full-Time 10-hour Day Shift to Providence Saint John's Ambulatory Breast Center in Santa Monica, CA.

We are seeking a RN Navigator who will coordinate cancer patient care and act as a patient advocate. This includes monitoring patients through screening/diagnostic evaluation, assisting them with navigation through healthcare systems, providing education and emotional care supportive to or restorative of life and well-being, and identification and support in the development of programs and services to enhance patient care and support. The RN Navigator will also provide education for general surgery patients.

In this position you will have the following responsibilities:

  • Identify patients newly diagnosed with cancer, with disease progression, or undergoing biopsies needing navigation. Assist these patients through screening, diagnosis, surgical treatment and transition to next steps, anticipating patient concerns, facilitating appointments, second opinions, and ensuring patients have access to clinical trials, clinical and educational materials to meet their needs. Serve as knowledgeable and reliable patient information source for diagnostic testing, treatment options, and long-term survivorship needs

  • Provide patient education and support for navigation and other patients of the Urologic Oncology Team, in the Cancer clinic

  • Serve as source of information and support for patients regarding holistic care including: physical, psychosocial, emotional, cultural sensitivity, language barriers, financial challenges, and spiritual support. Acts as a referral liaison for community practitioners. Assess patient needs and resources, assist patient with referrals to members of the Saint John’s Health Center team including, but not limited to, financial counseling, clinical social work, dietician and community support agencies

  • Attend site specific cancer conferences as appropriate to navigation patients, and contributes pertinent information relevant to patient care

  • Act as a crucial link for cancer research by demonstrating familiarity with available program-specific clinical trials, associated eligibility and exclusion criteria, and informed consent process. Assist physicians and research staff in identifying patients for available studies. Assist with communication and coordination of required research information

  • Act as an advocate on behalf of the patient, initiating and maintaining strong relationships with physicians, office managers and diagnostic services utilized in screening, treatment and marketing of cancer programs and services. Work with the clinical and leadership team to identify and resolve service issues

  • Communicate routinely with leadership team and reports on activities and outcomes proactively. Maintain required metrics for navigation service and program evaluation

  • Perform assessment/data collection:

  • Perform ongoing assessment of patients

  • Follow established policies and procedures to identify and reduce the risks of acquiring or transmitting infection

  • Identify learning needs for patient/significant other and demonstrates measures to meet those needs

  • Formulate goal directed plan of care based on patient outcomes:

  • Initiate, individualize, and modify plan of intervention

  • Develop and maintain strong relationships by identifying, anticipating and meeting/exceeding patient needs and expectations

  • Collaborate with other services to coordinate patient care

  • Coordination of transition between Med-Onc and urology in multidisciplinary cases like bladder cancer

  • Develop survivorship careplans as necessary

  • Provide BCG bladder installations when ordered for GU patients

  • Assist with triaging patient phone calls

  • Assist with PAP (patient assisted medication program)

  • Implement care based on department and hospital standards

  • Demonstrate competency in selected clinical skills including, but not limited to:

  • Administering PO, IM, SQ, and topical medications

  • Using equipment such as set-up for cystoscopy, uronav, spaceOAR and computer

  • May assist physicians, PAs, or NPs with examinations, procedures and other processes, including assisting surgeons with minor room procedures

  • Perform VS, assessment & medication reconcilliation when necessary

  • Demonstrate documentation on patient care records and unit forms

  • Complete Competency Plan for assigned job and department

  • Maintain clinical skills in all clinic areas

  • Perform dressing changes and suture removal

  • Manage patient’s surgery preparation in regards to education

  • Prioritize and respond to patient inquiries via phone or MyChart and documents same

  • Assist leadership team in evaluation of effectiveness of care

  • Assist leadership team in development and revision of educational materials for patients and families, to ensure accurate clinical content suitable for education level and language of cancer patients

  • Assist leadership team in evaluation of appropriate literature and peer review articles for development and implementation of evidence-based nursing interventions

  • Demonstrate applied knowledge when caring for selected patient populations including care appropriate to the age of patients served such as:

  • Age-specific principles of growth/development, psychosocial characteristics and interventions, communication skills and community resources

  • Disease specific knowledge of diagnosis, surgical interventions, medical oncology treatments (i.e. chemotherapy, biotherapy, anti-hormonal), radiation oncology treatments, survivorship, palliative care, end of life care and hospice

  • Demonstrate responsibility and accountability for own professional practice in regard to:

  • Attendance, punctuality and patient confidentiality

  • Positive public relation image and working relationships

  • Obtaining necessary knowledge and skill needed to maintain competence and certifications

  • Completing all mandatory requirements, including annual safety programs, CPR, infection prevention and annual physical

  • Willing to participate in cancer clinic nursing workflow when available and needed. See self as a Providence Saint John’s Cancer Clinic nurse and part of the clinic team. Priority is with Uro Onc team

  • Participate in the development and attainment of team, department and hospital goals including:

  • Attending and participating in team meetings

  • Volunteering or accepting assignments for projects

  • Delegate to appropriate patient care team members

  • Follow established protocols and procedures

  • Adhere to Mission, vision and team ground rules

  • Perform other job related duties as assigned or requested

Qualifications:

Required qualifications for this position include:

  • Current Registered Nurse license in the State of California required

  • Current BLS required

  • 3 years clinical nursing experience as Registered Nurse

  • Knowledge of the oncology disease process, related treatment protocols and common side effects and management

  • Ability to think critically, synthesize information, and communicate patient care needs in an organized and fluid manner

  • Ability to work independently and collaboratively, triage patients and keep physicians informed of patient status

  • Possesses basic computer skills: Microsoft Word, Excel, and Outlook

  • Ability to handle multiple tasks and appropriately prioritize

  • Ability to clearly communicate, both orally and in writing

  • Ability to maintain patient confidentiality and privacy in accordance to hospital policy

  • Ability to deal effectively with many people across a broad range of cultural backgrounds and ages

  • Ability to give constructive feedback, work with teams, teach staff and patients

  • Create and sustains respectful, inclusive, positive working relationships

  • Work with outside agencies on a routine basis, maintaining positive working relationships as well as assuring the financial viability of the hospital

  • Possesses flexibility in adapting to new and challenging situations

Preferred qualifications for this position include:

  • Bachelor's Degree in Nursing

  • Oncology Certified Nurse (OCN)

  • 3 years outpatient experience in a healthcare setting

  • Oncology experience
