Providence is calling a Full Time/Day Shift Chemical Dependency Counselor (Care Navigator) to Providence Cedars-Sinai Tarzana Medical Center in Tarzana, CA.
We are seeking a Chemical Dependency Counselor (Care Navigator) who will provide support to and build relationship with patients who have a chemical dependency diagnosis to ensure a successful transition from ED/hospital to the community and to reduce re-admission to the ED/Hospital. Support includes brief assessment of status of chemical dependency issues, assessment of readiness for change, identification of coverage for services, referral to appropriate services and follow-up to ensure services are received. Serves as a link between organizations serving persons with chemical dependency needs and the Medical Center to improve coordination of services.
In this position you will:
ASSESSMENT: Demonstrate knowledge and expertise in the assessment and treatment of substance abuse, mental capacity and incapacity, mental illness, adult protection, domestic violence, sexual and physical assault, psychosocial crisis, bereavement, and mental illness. Interview and observe the patient to assess the disease process of substance abuse and behavioral health issues; including physical, psychosocial, spiritual, education needs, cognitive status, and safety. Provide chemical dependency assessments standards and gathers data from family members and assess patient/family and resources for the purpose of developing plan of care
TREATMENT/INTERVENTION: Demonstrate effective clinical judgment, and provide appropriate therapeutic interventions, to address denial, progression of the diseases of addiction, substance abuse treatment, and behavioral health diagnoses. Communicat the treatment plan and after care plans to the patient, family and all providers, units, and referring agencies. Provide behavioral and medical intervention recommendations. Develop and coordinate the plan of care with both medical and nursing staff throughout the course of stay. Assist with the transfer or exparte of patients that need to receive treatment in another setting
DOCUMENTATION/INFORMATION MANAGEMENT: Document appropriately in EPIC. Treat all information and data within the scope of the position with complete confidentiality and security. Nursing managers and coordinators shall have access to the complete medical record of patients under their care for clinical or health care operation purposes, and may be reviewed by the Privacy Officer or designee. All members of the nursing staff shall have access to the complete medical record of patients who are on the floor or unit to which the staff member is assigned while on duty and working and all may be subject to review by the Privacy Officer or designee. Prepar quarterly CBISA statistical reports and submits to Community Benefit Department. Prepare an annual report on the program outcomes and impact
DISCHARGE PLANNING: Collaborat with physicians, unit team members, discharge planners, social workers to support substance abuse and behavioral health treatment needs. Facilitate referrals to PHSA agencies and community agencies. Receive hospital and community referrals and meet with patient in-person or by phone before they leave the hospital or Emergency Department whenever possible in order to explain the Navigation program. Phone contact for those who have not been able to be met with in-person. Ensure the patient is aware of and has follow up appointments with physician and ancillary providers and arrange transportation if needed. Monitor if follow-up appointments have been successful
EDUCATION: Facilitate conferences, training sessions, and lecture to community mental health personal to assist in expanding community understanding of substance abuse and mental health personnel to assist in expanding community understanding of mental and emotional problems as time allows: providing inservice training for Breakthrough, MHU, and PAMC. Attend and participate in patient care/treatment team meetings. Establish and help maintain relationships with the substance abuse and mental health providers. Coordinate and provide inservices. Increase knowledge and awareness of Providence Breakthrough Treatment programs. Provide information to PHSA staff about behavioral health services available to them and their families. Supervise social work students from UAA or other social work programs
CONSULTATION AND COLLABORATION: Build relationships between medical, behavioral health, and substance abuse units. Communicate interventions and treatment planning with all team members. Provide consultation to discharge planners for the facilitation of applications for follow-up care programs in and outside the PHSA system. Serve as a liaison between Hospital and public and private organizations serving persons with chemical dependency to improve coordination of car
RISK MANAGEMENT: Cooperates fully in all risk management activities and investigations

Required qualifications for this position includes:
Associate's Degree or higher in Clinical Psychology, Counseling Psychology, Social Work, or health related field
Must seek substance abuse counselor certification within two years of employment
Three (3) or more years experience in acute care settings to include substance abuse and dual diagnosis, thorough knowledge of disease progression of substance abuse, working with chemical dependency clients, and discharge planning ability to access appropriate referral sources
- Completion of CADC or CATC program
