Providence is calling a Discharge Planner (Per Diem/Day shift) to Providence Tarzana Medical Center in Tarzana, CA.

We are seeking a Discharge Planner who will be reporting to the Director of Case Management. Ensure timely discharge through early intervention and identification of post-hospital care needs. Ensure continuity if care needs are assessed and met for both inpatient and outpatients as needed, based on the age of the patient.

In this position you will:


  • Promote patient, physician, customer and employee satisfaction by personal example, treating all contacts with respect

  • Support and participate in activities that foster customer service

  • Communicate and follow the organizational chain of command for notification of patient care/service issues, when appropriate


  • Maintain required applicable licensure, certification and education

  • Attend and successfully completes general and department orientation, in-service programs and the annual key competencies and mandatory update requirements

  • Participate actively in department staff meetings

  • Demonstrate effective verbal and written communications

  • Communicate effectively in a clear, concise, understandable manner. Applicable for the populations: Infant, Pediatric, Adult, and Geriatric

  • Coordinate interfacilty transfers, communicating appropriately and promptly with hospital departments and other facilities as necessary

  • Initiate discharge plan with the case manager within 72 hours of admission

  • Update the discharge plan to reflect any change in the patient’s condition and need for alternative plans in conjunction with the case manager and physician

  • Act in coordination with the case manager, physician, nursing staff and other members of the health team to provide optimum continuing care after discharge

  • Completely and accurately documents all actions taken in the medical record

  • Participate in discharge planning

  • Round with Social Services and other hospital staff to develop discharge plans and discuss possible discharge problems. Rounds weekly in medical-surgical areas of hospital. Rounds weekly in maternal-child areas of hospital

  • Responsible for Skilled Nursing Facility, Durable Medical Equipment, Home Health and Acute Rehab referrals

  • Be a resource to hospital staff, patients, families, and physicians on community services and on agencies that provide outside services to patients

  • Demonstrate willingness & ability to float to areas within their scope of practice as necessary


Required qualifications for this position include:

  • Current LA City Fire Card within 30 days of hire

  • Knowledge of Medicare, Medi-Cal regulations

Preferred qualifications for this position include:

  • LVN

  • 3 years clinical nursing experience

  • 3 years discharge planning in an acute care facility
