Providence Health & Services Physical Therapist in Tarzana, California
Providence is calling a Physical Therapist (Per diem/ Day shift) to Providence Tarzana Medical Center in Tarzana, CA.
We are seeking a Physical Therapist reporting to the Director of Therapy Services, this position will be responsible for planning and implementing specific treatment programs for individual patients according to the principles and practices of physical therapy. The Physical Therapist (PT) collaborates with the interdisciplinary team and provides supervision of the Physical Therapy Assistant (PTA) and the Physical Therapy Aide (PT Aide).
In this position you will:

Communicate and follow the organizational chain of command for notification of patient care/service issues, when appropriate
Maintain required applicable licensure, certification and education
Attend and successfully complete general and department orientation, in-service programs and the annual key competencies and mandatory update requirements.
Participate actively in department staff meetings
Demonstrate effective verbal and written communications
Communicate effectively in a clear, concise, understandable manner. Applicable for the populations: Infant, Pediatric, Adolescents, Adults
Maintain knowledge of provision of individualized age specific care. Interacts with adult medical/surgical, geriatric and pediatric patients taking into consideration the needs of each specific age group (i.e. communicating in a careful, clear considerate manner, demonstrates appropriate instructions for ADL retraining, functional mobility training, WB status, safety precautions, cognitive retraining, therapeutic exercises, cognitive retraining, patient /family education for different cognitive levels and Physical abilities). Performs a complete evaluation including referring diagnosis, pertinent history, prior level of function, home/work/family situation, cognitive status, pain level and other subjective complaints, posture, sensation, neurological signs and special tests when appropriate, ROM, strength and functional status
Establish physical therapy goals (short and long term), plan of treatment and rehab potential. Formulate an appropriate discharge plan, re-evaluate patient and adjusts goals and plan when necessary (weekly on acute and rehab, monthly in outpatient)
Implement treatment plan and demonstrate competency in procedures including bed mobility, transfer training, pre-gait and gait training, therapeutic exercise, massage, mobilization, neuromuscular facilitation, neurodevelopmental training and patient/family education; modalities and use of equipment including DME, prosthetics/orthotics, thermal modalities, ultrasound, electrical stimulation, whirlpool, lavage, topical medications, traction, hoyer lift, CPM and exercise equipment
Identify and observe all patient precautions including total hip and total knee replacement, spinal, fall risk and weight bearing status precautions
Identify and observe equipment precautions including CPM use, brace use, oxygen use, intermittent pressure pumps, drains, catheters, IV’s and other lines to the patient
Identify and observe all precautions when using modalities
Complete daily progress notes after each patient treatment. Notes are complete, thorough, legible, timely and accurately reflect the treatment provided and the associated charges. Co-sign daily progress notes and weekly and discharge summaries completed by the PTLA, PTALA, PTA, and PT aide as required by California Physical Therapy Regulations and department policy and procedure
Recognize indications/contraindications and normal/abnormal responses to treatment and modify the treatment program appropriately as indicated by the patient’s response
Initiate implementation of solutions after considering all available information, raising relevant questions, seeking input from other team members and developing a clinical justification for solutions chosen
Demonstrate time management skills by achieving a productivity standard on average of 24 units of service in an 8-hour patient care shift. Prioritize commitments, manages multiple tasks simultaneously and tolerate inconsistencies in the health-care environment with a positive and professional attitude
Demonstrate willingness and ability to float to areas within their scope of practice as necessary
Participate in case conferences and discharge planning activities
Coordinate with a CCS Case Manager for referral to a CCS-Paneled provider or CCS Medical Therapy Unit (MTU) for the patient who may continue to require occupational therapy (or physical therapy) services after hospital discharge
Required qualifications for this position include:
Bachelor's Degree in Physical Therapy
Current LA City Fire Card within 30 days of hire
Current license or license applicant in the State of California as a Physical Therapist
Ability to effectively evaluate and treat patients with a variety of diagnoses
Preferred qualifications for this position include:
- 1 year acute care experience
