We are seeking a Activities Planner/Social Service Designee who will be responsible for overseeing and leading the Activity Programs & Social Service Support as required by Title 22 and Medicare Guidelines.
In this position you will:
Develop, maintain, and ensure compliance of monthly Activity Calendars for TCC/TCU
Activities shall reflect the patients interests and abilities
As appropriate, individual Activities will provide sensory stimulation, reality awareness, group interaction, and/or personal achievement
Activities will encourage and allow patients to make their own decisions and will allow them to operate as independently as they are able
Document initial assessment note within 7 days of admission to the facility and provide periodic updates as appropriate, no less then quarterly
Activities Program will establish an active and meaningful Volunteer Program
Activities Program Planner will communicate his/her duties to the staff, providing periodic in-services, updates, and/or presentations
Promotes community involvement for the staff, soliciting community group presentations, entertainment, and/or education
Social Service Duties:
Provide direct service intervention to individual patients, couples, families and groups to include Psychosocial assessments, supportive therapy, discharge planning, and community resource referrals
Actively participates/facilitates patient care rounds/case conferences
Proactively identifies unmet patient needs in area of service and recommends programs/services to meet those needs
Review charts and completes for appropriateness, timeliness of intervention, identification of issues, treatment plan and follow-up
Prioritize caseload by completing assigned work with minimal assistance and supervision
Serve as a patient advocate; assists patients with advanced directives and maintains current knowledge on ethical issues relative to inpatient care
Proactively identifies areas of unmet needs in relation to patient's rights
Through in-service, Staff Meetings, and/or IDT meetings, maintains a positive working relationship with staff, physicians and local agencies and providers; seeks assistance in resolving issues/conflicts
Participate in hospital and community programs and committees pertinent to areas of service
Assist the department in special projects as assigned
Demonstrate leadership and independence in areas of focus
Is visible to staff and facilitates staff interaction to better serve the patient's needs
Serve as a role model
Maintain a positive approach to workload and changes within workplace
Be responsible to orient staff and volunteers as assigned
Document all patient care according to policy
Ensure confidentiality of all patient information
Assume responsibility and accountability for own professional growth and practice. This includes maintaining current certifications and completing all required in-service education
Attend all assigned staff development activities for department including a minimum attendance of 75% at all scheduled work time staff meetings
Provide in/conducts in-service education, as requested
Participate as requested in agency Improving Organizational Performance Activities
Promote use of the agency C.A.R.E. methodology in problem solving

Required qualifications for this position includes:
Coursework/Training: Activities Designee Training from an approved program
Coursework/Training: Social Services Designee Training from an approved program
Activities Designee Certification from an approved program
Social Services Designee Certification from an approved program
Consultation by an approved LCSW
Two (2) years of Experience in the post acute setting (SNF)
