In this position you will:
Be responsible for ensuring that physician ordered continuous care services are coordinated and delivered appropriately to hospice patients/families.
May provide care to premature infants, infants, pediatrics, adolescents, young, middle and older adults.
Assist with overflow of CHHA scheduling across each clinical team.
Also be responsible for supply management with Medline and approval of ordered supplies.
Perform all duties in a manner that promotes team concepts and reflects the Providence Health & Services mission, philosophy, and core values ofcompassion, dignity, justice, excellence and integrity.
Required qualifications for this position include:
LVN with current licensure to practice professional nursing in the state of California
Graduate of NLN accredited school of nursing
Current Basic Life Support ( BLS ) certification
Valid driver's license with an automobile that is in good working order and insured in accordance with state and/or Providence Health & Services / TrinityCare Hospice requirements
Proven ability to work within an interdisciplinary setting
Comply with accepted professional standards and practice
Demonstrate excellent observation, verbal and written skills
Demonstrate good nursing judgement
Understand hospice philosophy and issues of death / dying
Working knowledge of electronic forms of communication and documentation (i.e, Microsoft Outlook, Microsoft Word)
Preferred qualification for this position include:
- Two years specific hospice experience
