Home Care Intake Specialist (LVN) - Providence Little Company Of Mary Home Health
Under the direction of the Administrator of Providence Home Care and the direct supervision of the Supervisor Community Liaisons, facilitates referrals, coordinates staff and assists patients and families with transitions from hospital to home. The Intake Specialist communicates with physician, hospital staff, patients, caregivers, and agency staff in order to provide continuity of care.
