Seeking a full-time Hospice and Palliative Care nurse practitioner to join a high-functioning, interdisciplinary team in Torrance, California. Providence TrinityCare Hospice provides a full range of palliative care, hospice care and compassionate support to patients and their families throughout Southern California. The Hospice and Palliative Care nurse practitioner will function independently as an advanced nursing professional and will have the responsibility to coordinate and assess care needs, create and modify the plan of care and make appropriate referrals for patients in collaboration with the hospice and palliative care interdisciplinary team.
