We are seeking a Patient Safety Program Manager South Bay Community Service who will design, implement, manage and evaluate the South Bay Community patient safety program. The program manager may be primarily located at one of the acute ministries within the South Bay (San Pedro or Torrance) but will be expected to work throughout the continuum of the South Bay Community.
Using assessment tools and patient safety data, the Program Manager is responsible for identifying and implementing improvement initiatives. The Program Manager will analyze clinical processes, identify potential risks for patients and develop strategies to maximize safety, effectiveness and efficiency. Along with the Patient Safety Officer, the Program Manager develop the education regarding patient safety, and works to influence leaders to achieve measurable improvements in patient safety that will support the sustainability of reductions in medical/healthcare errors and other factors that contribute to unintended adverse patient outcomes.
In this position you will:
In conjunction with the Patient Safety Officer and other Patient Safety Program Manager(s), plan, develop, implement evaluate and improve the Patient Safety Program within the South Bay Community which consists of two general acute care hospitals, two post-acute skilled nursing facilities, acute rehab, acute and outpatient psych, chemical dependency recovery and several affiliated off-site locations.
In conjunction with Risk Management, receive referrals and intervene with patient and/or family to resolve a grievance vs. Hospital and/or personnel in order to eliminate or diminish potential liability claim.
Co-lead the South Bay Community journey towards a High Reliability Organization, including representing the South Bay Community at System and Regional meetings.
Assist with the preparation of reports for the South Bay Community Executives on the occurrence of known medical and health care errors, identified near misses, dangerous conditions within the South Bay Community, as well as actions taken and barriers to the implementation of safety program.
Review statistical analysis of the frequency and causes of general categories and specific types of adverse incidents causing injury to patients and recommends appropriate measures to minimize these incidents.
Oversee the collection and dispersion of information related to patient safety through monitoring and reporting performance relative to patient safety indicators (metrics).
Establish metrics for the South Bay Community of patient safety progress.
Review and complete follow-up and investigation as needed on events in order to comply with State and Regulatory Agency mandatory reporting requirements, including but not limited to root cause analysis, failure mode effects analysis, and other methodologies as appropriate.
Collaborate with clinical staff to develop, implement, monitor and champion performance improvement processes and policies related to key patient safety issues which may include championing work related to certain disease states (e.g. sepsis, mortality reduction, etc.).
Coordinate efforts of various departments and stakeholders related to patient safety, share the experience and learnings with others through professional presentations.
Present educational material to all employees upon orientation, at yearly educational updates, and additionally requested in order to satisfy continuing education requirements related to patient safety.
Lead the development of policies and procedures related to patient safety.
Manage the Culture of Safety survey process ensuring action plans are created, implemented and maintained.
Manage the South Bay Community’s Safety Rounds, gathering information and data, making recommendations for improvement and implementing these actions gleaned by the Safety Rounds.
Support and encourage error reporting throughout the organization through non-punitive error reporting systems.
Required qualifications for this position include:
Bachelor's Degree in Nursing or Healthcare related field.
5 years experience in an acute care hospital.
Knowledge of patient safety regulations, state and federal laws.
Knowledge of working with complex statistical data and assessment tools.
Knowledge of methods, processes, procedures and techniques for conducting patient safety activities.
Cognitive thinking skills.

Preferred qualifications for this position include:
Master's Degree in Nursing or Healthcare related field.
Current California Registered Nurse License or other healthcare equivalent
Certified Professional in Patient Safety (CPPS)
2 years patient safety experience.
