Providence Senior Community Health Worker in Torrance, California

Description

The Senior Community Health Worker will screen patients and/or community residents in the community, medical clinics, inpatient, or emergency department settings for risk factors that may impact an individual's well-being. The Senior Community Health Worker will then refer patients/community residents to appropriate programs and resources in the community and provide additional follow-up to make sure patient/community resident needs are met. In addition to direct communication with patients/community residents, the Senior Community Health Worker will organize documents, spreadsheets, and provide relevant documentation for each encounter with patients/community residents.

Required Qualifications:

  • Education to meet certification, license or registration requirement.

  • Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request.

  • 1 year - Experience in a combination Community Health Worker I and II position, or 2 years related community outreach, enrollment, education or other community health administrative experience, or Bachelors degree in lieu of experience.
