Providence is calling a Clinical Social Worker, MSW to our Community Health Outreach (Temporary - Full time/Day shift) based at Holy Cross Medical Center in Mission Hills, CA

We are seeking a Clinical Social Worker MSW Community Health who will provide interventions to remove psychosocial barriers preventing safe and timely transition through the continuum of care. The MSW collaborates with members of the Emergency Department healthcare team, patient/family and community liaisons to assure psychosocial and socioeconomic needs are addressed in order to enhance outcomes and patient satisfaction. The MSW also provides training related to follow up functions identified in a Social Work assessment within the scope of Community Health Worker job functions. This position is grant funded and will end December 2021.

In this position you will:

  • Provide age appropriate social service intervention proactively and upon request to individual patients, couples, families and groups to include: Psychosocial assessments, supportive therapy, coordination or care, discharge planning and community resource referrals

  • Review charts and completes intervention with emphasis on identification of issues related to psychosocial assessment, treatment plan, intervention/referral and follow-up

  • Actively participate/facilitate patient care rounds/case conferences

  • Proactively identify unmet patient needs in area of service and initiates programs/services/in-services as needed

  • Knowledgeable and helpful in assisting patients with advanced directives. Is current on ethical issues in patient care areas

  • Proactively identify areas of unmet need in relation to patient rights and follows through with appropriate intervention

  • Maintain knowledge of policies related to clinical and organizational ethics

  • Recognize indicators of abuse and neglect; consistently follows mandatory reporting guidelines

  • Maintain up-to-date knowledge of reporting responsibilities and shares with healthcare team evidenced by providing employee in-services as the need is identified

  • Proactively participate in the charity programs by referring all appropriate candidates

  • Collaborate with other department staff relative to shared cases in a timely manner

  • Attend the majority of staff meetings and participates in departmental functions. Communicate when unable to attend

  • Actively participate in hospital and community programs and committees pertinent to areas of service

  • Concentrate on area of assignment but maintains the flexibility to perform routine duties in any area of the hospital or in community settings

  • Assist the Community Health department in special projects, as assigned

  • Accept on-call and/or weekend assignments as requested or required by the department

  • Documentation/Information Management:

  • Demonstrate working knowledge of the hospital Information System

  • Practice professional confidentiality and serves as a role model for others

  • Document appropriate information pertaining to the individual patient in a specific, accurate and thorough manner to allow for follow-up problem/resolution

  • Maintain proficiency in computer and written documentation as defined by department guidelines

  • Keep an updated personnel record per facility policy (i.e., PPD, Educational Record)

  • Leadership and Supervision:

  • Practice in a professional and knowledgeable manner

  • Serve as an internal and Community Health resource for others regarding psychosocial issues in patient care

  • Act a role model and maintains a positive approach to change

  • Practice effective two-way communication

  • Provide referrals for psychosocial issues to healthcare team members (i.e., referrals to EAP)

  • Demonstrate the ability to make appropriate decisions within the scope of responsibility

  • Share valuable information with co-workers and peers, including training of community health workers in their roles and responsibilities related to follow up functions identified by the MSW assessment

  • May provide supervision to MSW students participating in internship programs. This position is supervised by the Manager Community Health

  • Development:

  • Complete mandatory continuing education requirements for relicensure, as appropriate, independently

  • Demonstrate interest in professional growth through attendance of workshops and seminars

  • Attend LCMSA in-services and other educational activities to maintain current knowledge of practice and technology that is utilized in the role

  • Provide in-services to others on the healthcare team (i.e., Community Health Workers)

  • Provide orientation to new employees/students, as requested

  • Provide clinical consultation to staff/physicians regarding the psychosocial aspects of patient care, asisting others to improve their knowledge base

  • Set valuable and attainable annual objectives

  • Performance Improvement:

  • Uphold and strive to meet the Continuum/System goals and objectives - accepting change positively

  • Participate performance improvement in patient care related issues

  • Identify issues, seek resolution and ensure follow through linkage to community resources

  • Seek assistance/input when needed from manager/director, as needed

  • Budgetary:

  • Utilize time and supplies wisely

  • Meet deadlines for assigned duties

  • Attend meetings on time

  • Is flexible within the department to assist others/provide coverage

  • May identify areas of potential savings

  • Adapt working hours to job requirements

  • As assigned within general scope of job, training and licensure

  • Participate in on-call, after-hours rotation

Qualifications:

Required qualifications for this position include:

  • Master's Degree in Social Work (MSW), from of an accredited program

  • 1 year recent experience in acute care inpatient setting or community based organization with responsibility for assessment of patient population and linkage to community resources in area of service.

  • Computer experience/literacy as needed for the job

  • Requires advanced communication skills to communicate effectively with patients, hospital staff, physicians and the community

  • Requires flexibility and strong self-motivation/organizational skills to prioritize work and make decisions with minimal supervision

  • Requires a courteous and cooperative service-oriented demeanor

Preferred qualifications for this position include:

  • Experience working with drug/substance abuse population

  • Discharge planning experience

  • Knowledgeable of community resources with care coordination

  • Experience working with EPIC and MyChart or other electronic medical records systems

  • Bilingual in Spanish
